For incoming Kindergarteners
A child must be 5 years old by September 1 st and developmentally ready for kindergarten. A developmental readiness test is usually administered to interested students in the early part of the year by appointment. An interest form should be filled out as the first step.
For Grades 1 through 8
A child must be 6 years old by September 1 st for grade 1 and developmentally ready. The date requirement may be waived at the discretion of the principal. A entrance assessment is usually given to interested students upon completing the interest form. Copies of recent report cards and standardized tests will be requested for review.
Upon acceptance-
Registration Papers
- a copy of the birth certificate
- a copy of the baptismal certificate( if available)
- current school test scores
- a completed registration form
- a completed emergency card
- a completed tuition agreement along with fees
- updated immunization records
Priority Placement
Priority for admission is given to siblings and registered parishioners of OLPH, and surrounding area parishes. Other Christian denominations are welcome to apply.
Waiting List Procedure
Parents may place their children on the O.L.P.H. waiting list at any time by calling the school office.
IMMUNIZATION
No child may be admitted as a student of a school unless he/she has complied with California immunization requirements.
Polio - 4 doses , 3 doses are enough if at least one was given after the 2nd birthday.
DTP/DtaP or combination of DtaP with DT – 5 doses at any age . 4 doses meet requirements for ages 4-8 if at least one was given on or after the 4 th birthday.
For age 9 years and older – Td, DT, or DTP, DtaP or any combination of these, 4 doses at any age.
MMR (Measles, Mumps, Rubella) - 2 doses , MMR must be given on or after the first birthday.
Hepatitis B - 3 doses.
Varicella (chickenpox) – 1 dose for kindergarten entry. If the student has had chickenpox, the child’s doctor must verify this.
All students entering kindergarten/first grade must have a Report of Health Check-Up for School Entry (DHS:MCH-77) completed by your family doctor before school entrance. This health examination must be completed within the 18 months prior to entering first grade.
Families may exempt a child from the immunization requirements for personal beliefs or medical reasons. Please contact the school for details.
TUITION PLANS
OLPH School offers two (2) tuition plans, both with discounts for families with more than one child.
Plan A : Active Participating Catholics receive a discounted tuition rate if they are registered in a Catholic parish of their choice, attend weekly liturgy, and financially support parish life through the regular use of weekly envelopes. OLPH School believes the amount you give is a personal matter, that when prayerfully considered, will reflect a generous Christian spirit of giving a portion of your sustenance.
OLPH School does verify parish registrations and participation (not specific dollar amounts) on a regular basis. A change in tuition plans may be recommended after reviews are complete. We continue to encourage all families to a fuller participation in their Catholic faith for the benefit of their children and their own personal growth as Christians.
Plan B is offered to those whose affiliation is other than Catholic or those Catholic families who opt not to participate in parish life. We realize there are many reasons why families choose a Catholic education. WE, at OLPH, continue to appreciate and welcome our families from other various denominations. We respect each family’s choice in partnering with OLPH School to provide each child’s academic and social needs in a safe, challenging, and loving environment. We continue to extend this invitation and welcome these families as our own.
TUITION PAYMENT OPTIONS -
Families may choose to make payments through automatic electronic withdrawal or pay in full with a discount.
